- Name on the account – this should be the same as in our letter. If it isn’t, tell us why it’s changed in the form
- Sort code
- Account number

Check our letter to see if you also need
Your bank statement – just the first page, from a statement received within the last three months. It needs to show the bank or building society logo, full account name, sort code and account number
Photo ID – a copy of your current driving licence or passport
Please only upload PDF, JPEG and JPG files no bigger than 4MB each.
Once we’ve checked we have everything we need, we’ll make your payment within 30 days.
